- Pre-Admission Procedure
- No patient will be directly admitted into Dementia Village.
- A doctor and the Patient Relations manager will have to visit the patient at the address of their current stay and evaluate the patient’s conditions before admission.
- After evaluation, the doctor will then forward his recommendation to the admission panel who will take the call on whether to admit the patient or not.
- The guardian of the patient or the person responsible for the admission of the patient will be advised to visit the dementia village before admitting the patient.
- If the patient is suitable for admission and the patient’s guardians goes against admission, the reason towards the same needs to be filed.
- Admission Procedure into Dementia Village
- Admission of the patient into Dementia Village will have to be approved by the admissions panel.
- Once the necessary documents have been filled out, the General manager must review the filled information to ensure that all the relevant data is filled.
- At the time of admission , the patient’s relevant healthcare records shall be submitted to the healthcare incharge.
- The patient’s daily requirement of cloths and other items shall be handed over to the personal incharge.
- No Patient may be admitted into the Dementia Village with a duly filled Admission forms .
- At the time of admission, the patient’s healthcare record, which is maintained by NIKISA is filled in front of the patient’s caretakers by the present doctor and nurses.
